Issue Resolution Group (IRG)
Programme Change Request CR062: Issue Resolution Group (IRG) – MHHS Programme Governance Framework Update introduces a new governance group to manage and resolve material issues which impact Programme delivery.
On 2 July 2025, the Programme Steering Group (PSG) approved CR062 and the IRG Terms of Reference, which you can view below. This means that a new issue resolution governance group will be formed for Milestone (M10), that will run until M15.
Issue Resolution Group (IRG) Terms of Reference
The IRG responsibilities are to be the ‘resolver of last resort’ and the group may be invoked by the Senior Responsible Owner (SRO) to expedite the resolution of an issue that has exceeded the service management resolution thresholds and is sufficiently material that:
a) the Exit Date for Early Life Support (ELS) is compromised or
b) the Migration schedule is compromised placing M15 at risk or
c) an issue leads to consumer detriment with no agreed rectification plan in place
IRG is separate and will not replicate Service management. IRG members must be technically knowledgeable of their system design and empowered to support IRG decision-making.
An IRG meeting may be convened at short notice, as and when issues are identified, although as much notice as possible will be given.


What happens next?
The governance group requires nominations from participants to fulfil industry membership. The nomination process has opened and members appointments should conclude by late August 2025.
Please download the nominations form below and email it to PMO@mhhsprogramme.co.uk by 17:00 on Friday 25 July 2025. The Programme will then validate and process submissions.

Once the group and members are established a series of rehearsals will be conducted before Milestone 10 (M10), to finalise communications and ensure that members attend the meeting at short notice and practice issue resolution.
Please note that IRG is separate to central party Service Management.
